Official Contest Rules
- Prize – One (1) winner (Prize Winner) will be randomly drawn from entries received to win a 8 day/7 night trip for two to the Grand Cayman, Carefree in the Caribbean (“Charity Raffle”).
- Prize Description – The Prize Winner will win a 8 day/7 night beachfront studio all inclusive stay (meals on property) for two (2) at the Reef Resort, Grand Cayman Island for the period of June 16, 2012 through June 23, 2012. The prize includes a $1,000 airlines gift certificate and round trip transfers from the Cayman International Airport. The approximate retail value of the prize is $4,754. The Prize Winner will be required to meet all terms and conditions of Travel and Cruise Professionals as well as Prize Winner’s selected airline rules and regulations. The Prize Winner will be responsible for all other costs and expenses associated with the prize, including but not limited to taxes, gratuities, and entertainment costs.
- Term – The Charity Raffle will commence on December 21, 2011. The Prize Winner will be randomly drawn at 12:00 noon on February 29, 2012 at the offices of the official sponsor of the Charity Raffle, Diabetes Association of Atlanta, Inc. (DAA), 100 Edgewood Avenue, Suite 1004, Atlanta, GA 30303. You do not have to be present to win.
- Eligibility – Subject to the additional restrictions below, the Charity Raffle is open to residents of the State of Georgia who are 18 years or older at the time of entry. Persons can enter as often as they desire. Employees and the Board of Directors of DAA shall not be eligible to win the Charity Raffle. Family members of employees and family members of the DAA Board of Directors are eligible to win.
- Entry Instructions- There are several ways to enter the Charity Raffle.
- Visit www.diabetesatlanta.org and make an online donation to Diabetes Association of Atlanta, Inc. in the amount of $25 per ticket via credit card or PayPal. All online donations must be received by 5:00 p.m. February 28 , 2012.
- b. Mail your donation of $25 per ticket (cash, credit card, or check) to Diabetes Association of Atlanta, Inc. at 100 Edgewood Avenue, Suite 1004, Atlanta, GA 30303. All mailed donations must be received by 5:00 p.m. February 28, 2012.
- c. Purchase ticket from a DAA Board member or Employee for $25, cash or check.
- A bundle of five (5) tickets may be purchased together for $100.
- Odds of Winning – The price per entry is $25.00. A maximum of 1,500 entries will be sold. Sales will cease when that number of qualified entries is reached. Any requests and payments received following the cut off will be returned to the sender. There shall be only one (1) Prize Winner. Odds of winning will depend on the number of eligible entries received not to exceed 1,500 to 1. DAA will not be responsible for any incomplete, lost, late, stolen, misdirected, illegible, or unintelligible entries, or for entries which were not received due to technical, computer or transmission failures resulting from any malfunction of hardware or software. It will be the sole right of DAA to determine the validity of any entry and DAA reserves the right to disqualify any person who tampers or abuses the Charity Raffle. Each entry will receive and bear a unique number. All entries become the sole property of DAA.
- 7. By participating in this raffle, each participant releases DAA and its respective directors, officers, employees and agents from any and all liability for injuries, losses or damages of any kind caused by any prize or resulting from acceptance, possession, use or misuse of any prize, and each winner agrees to indemnify DAA and its respective directors, officers, employees and agents harmless from any and all losses, damages, costs, expenses, rights, claims and actions of any kind arising in connection with or as a result of the winner’s acceptance or use of any prize. An IRS form 1099 will be issued to the Prize Winner for the value of the prize. Prize Winner is responsible for all applicable taxes and expenses incurred by winning the Charity Raffle. Donations made in the Charity Raffle are not tax deductible.
- Notification to Prize Winner – DAA will notify Prize Winner via telephone and overnight delivery service on or before March 2, 2012. Prize Winner must sign and return an Affidavit of Eligibility and Liability and Publicity Release no later than March 20, 2012 as a condition of receiving the Prize. In the event Prize Winner fails to complete such Affidavit and Release, the Prize shall be forfeited and the Prize Winner disqualified. Prize Winner may also waive his right to receive the Prize. In the event of forfeiture or waiver in receiving the Prize, DAA may award the Prize to another randomly drawn winner from the remaining entries.
